No, Vitality Club is not an approved NDIS provider, but if you are self-managed or plan-managed, you can choose to use our services to help achieve your goals.
Yes. Approved NDIS providers must not charge more than the amounts laid out in the NDIS Price Guide. To see the latest NDIS Price Guide, visit the NDIS website.
If you do not already have an NDIS Plan at the age of 65, you are no longer eligible for funding. However, if you are 65 or over, you may qualify for the Commonwealth Home Support Program or Home Care Packages.
You will need to provide information about yourself to help the National Disability Insurance Agency (NDIA) assess your eligibility. This is called an Access Request. You can also permit someone you trust to provide information on your behalf. Access Request Forms are available here.
You can follow this link to the NDIS website, which provides information on the types of evidence required. Local Area Coordinators can assist you with the process, however they cannot complete the Access Request Form or collect evidence on your behalf.
The NDIS is fully funded by the Australian Government, and you will not need to pay any out of pocket fees.

Our allied health team can help you prepare for your planning meeting by providing a comprehensive assessment and care plan. Our care plan objectively outlines your current supports, how you manage everyday activities, the goals you want to achieve and specialist recommendations on the funding and services required to help you reach these goals. Many participants find it helpful to have a report written by an Allied Health professional to supplement their planning.
