A GP Management Plan (GPMP) is designed to help GPs care for people with chronic diseases or conditions as part of a broad health service team.
A chronic medical condition has been (or is likely to be) present for six months or longer. For example asthma, cancer, cardiovascular disease, diabetes or musculoskeletal conditions.
If you are eligible, this plan entitles you to a total of five subsidised Allied Health sessions per calendar year. You can choose to use your plan for one type of service or access a range depending on the services you require.
CDMP eligibility is determined by your GP, based on your medical condition and care needs. This program is subsidised through Medicare and can be accessed in addition to services through My Aged Care and the NDIS.
Speak to your GP about creating a GP Management Plan. Your GP will conduct an assessment and inform you of your eligibility.
