Government Supported Home Care Parramatta

To understand what you are entitled to, your first step is to visit the government’s My Aged Care website or call 1800 200 422.

My Aged Care will refer you to a Regional Assessment Service (RAS) or to the Aged Care Assessment Team (ACAT) for an assessment. Depending on the outcome of your assessment, you may be allocated a Home Care Package (HCP) or provided access to Commonwealth Home Support Program (CHSP) services.

If you have been allocated a Home Care Package, we encourage you to revisit My Aged Care to compare service providers, prices and care plans. If you have not been allocated government support but would still like to discuss home care packages, our team can also assist you with private home care services.

By reviewing and comparing providers, you will gain a better understanding of the Aged Care industry. This will help you select the provider that can best cater to your needs. We encourage all Australian Seniors and their carers to do their homework by exploring the My Aged Care website and speaking to providers. Selecting a provider is a big decision for the whole family. Assessing all options will help you feel confident and comfortable that you’ve found the best provider for you!.

Vitality Club Home Care Packages

All Vitality Club clients are allocated a dedicated home care Case Manager who will play an active role in ensuring your care needs are consistently met.

Your Case Manager will take the time to get to know you, your goals and your requirements. From here, they will work with you to create a personalised care plan, with the relevant services available through our Home Care Package program. These services could include domestic assistance, in-home Podiatry, social support or exercise classes.

As your needs and goals evolve and change, your personalised care plan and the services you receive will also change. This ensures you are always getting the most out of our services and the subsidies that have been allocated to you. 

Once set up, your Home Care Package services are ongoing but can be paused at any stage.
